We have a drugs folder, we have every drug we keep in alphabetical order so if we give our say duraphat toothpaste I have a sheet for duraphat I then write
Date prescribed
Name
Dob and CHI number
Unique Nhs number (I'm hospital)
How many given
How manta left in stock
Who's prescribed
Then my initials as the person singing it out
it's then checked along with our drugs every couple weeks to ensure it matches and to allow an order for restock. You could make up 1 sheet and just write what it would be for when you print it. I usually get the clinician to initial it also
