Yes. If you chart it on the baseline it will chart as caries only. you have go onto the plan and chart from there and then it shall be added onto the treatment plan. When I use to R4 I use to write onto the notes the tooth with the surfaces with caries and then just put it straight into the plan. Says you charting twice. But I suppose it all depends on how the dentist wants you to chart!

Hi I’ve not long started as a trainee dental nurse, I’m struggling with the charting R4 software, I struggle what to put on the baseline then on the plan? Any tips? Say I the pt had mesial occlusal caries would it go on the baseline then have to add to the plan only if it’s going to be repaired?
