Conscious Sedation - Nitrous Oxide

Hi,

We have a clinician recently done the conscious sedation course, we are about to set the whole thing up within practice, I was reading through the SAAD guidelines and believe the paper work side of things we need the following:

1) Sedation Policy
2) Sedation Consent Form
3) Pre-Op Instructions
4) Post - Op instructions
5) Monitoring Record e.g. pulse oximetry, blood pressure etc
6) Sedation Course - Nurse

I'm guessing we would need to do an audit? how often? where could I find an example of a conscious sedation audit?

Is there anything more I need to put in place before allowing conscious sedation to be carried out within practice?

Any help/advice would be greatly appreciated
